IMPORTANT NOTE

Each dental office listed here has agreed in writing to accept the CSEA EBF Fee Schedule as payment in full for services covered by our dental plans. In addition, these dentists have agreed that for any treatment not covered by our Plan, they will discuss the cost with the covered employee before providing services that result in out-of-pocket expenses.

Specialists within participating general practices may have the right to bill members for the difference between the specialist's customary charge for the service and the allowance which the CSEA Employee Benefit Fund pays under the applicable Fund dental plan. The specialist must inform the Fund and the member that he/she will not be accepting the allowance as payment in full and must provide proof of specialty status to the Fund. The member will be asked to sign an acknowledgement of their financial responsibility for the procedures provided by the specialist prior to service.
