in locating a CSEA EBF panel provider in your area.
What to do after you have located the provider you
wish to use:
If you have already enrolled in the CSEA EBF Vision
Care Plan and verified your eligibility, you may now
select a provider. All you need to do is make an
appointment for your eye examination. The
participating provider will obtain an authorization
number, and no paperwork or voucher will be necessary.
This listing is continually subject to change. It is
your responsibility to contact the vision provider's
office and confirm that they are still a plan
